孙撰。四卷。宋、齐二朝史书有《州郡志》，北魏史书也有《地形志》。但自东晋十六国以来，侨州郡县，朝南暮北，旋有旋亡，资料紊乱；其后北齐、北周、陈与南、北诸正史都缺地理志。洪氏以《梁书》纪传为主，辅以《南齐书》、《魏书》、《南史》、《北史》、《北齐书》、《周书》、《陈书》、《隋书》以及《元和郡县志》、《太平寰宇记》、《舆地广记》、《舆地纪胜》、《通典》、《通考》、《通鉴》、《地理通释》等书中的资料，略仿宋齐二志之例撰为是书，详列魏、隋二注之文，其州郡县因宋齐旧制而未经分割者，准宋齐二志为多。其魏梁之间归属变迁，则从魏隋二注，其分置不见于本书与魏隋二注的，则引他书以为佐证，其间实土、虚名，以及一名二地或一地二名者，一一条述。于名山大川、旧关重镇、馆殿台阁、宫阙园陵，都作了详实记载。有《史学丛书》本。收入《二十五史补编》。
